I got my OPT in 2000 after undergraduate. Now, I am trying to apply for OPT again after graduate school and the I-765 (application form for OPT) asks whether I have ever applied for OPT? If yes, to provide necessary documentation. I do not have my EAD card from 7 years back. What document can I provide? Or should I just say that I never applied for OPT as I have gone back to Nepal for a year in between? Or should I just write a nice letter saying that I do not have the EAD card anymore and just send the copy of my degree as a proof that I applied in 2000.

Your school should advise you on the documents required for OPT. However you may submit a copy of your old I-20 which has OPT authorization. If not you may write a letter with the details. Please get more advice for your school! Usually OPT is granted regardless such evidence is produced or not but please do not lie in your application!

Yes, Do not write "No". Just provide a copy of the OPT I-20 if you have it, or get if from your previous school if they have it in their records. If not, then just consult with your international advisor. I'm sure they will have a solution to your problem. Good Luck!
